Authorized Signatures
Please make sure this form is kept up to date in the Building Management Office and submit a new form whenever there are changes.
These individuals are authorized to sign for Property Removal Passes, after-hours access, key orders, overtime HVAC, graphics requests and access card additions/deletions. Please mark the appropriate box to indicate which items the person is authorized to sign.
Emergency Notification
This form lists your company's contacts in case of an emergency. Please make sure this form is kept up to date in the Building Management Office and submit a new form whenever there are contact changes.
These individuals will be contacted whenever there is a bomb threat or similar emergency at 101 California. The decision to vacate your premises will be at the discretion of these individuals. Please list them in the order in which they should be called.
Floor Wardens
Please make sure this form is kept up to date in the Building Management Office and submit a new form whenever there are changes.
These individuals are designated by your firm to respond effectively and assist other employees in the event of an emergency situation. These individuals are required to attend a Life Safety Seminar provided by the Building Management Office, and wear a vest provided by the Building Management Office to identify them in an emergency.
List of Physically Impaired Persons
Please make sure this form is kept up to date in the Building Management Office and submit a new form whenever there are changes.
This is a list of any physically impaired persons in your office that may need extra attention in the case of an emergency. Please let the Building Management Office know of these cases, even if it is a temporary situation (i.e. a broken leg in a cast).
Tenant Contact Information
This form lists your company's executive, day, after hours and invoice contacts. Please make sure this form is kept up to date in the Building Management Office and submit a new form whenever there are contact changes.
Please note that one individual may serve as the contact in several areas. It is also important to remember that the Executive and Primary Day Contact must be located in 101 California
Executive Contacts: These individuals are designated as the primary contacts in the event of a building emergency at 101 California, (i.e. earthquake, fire, bomb threat, etc.) Due to the extreme importance of fast and efficient information, it is imperative that this information is current and accurate. The name you provide should have the authority to make decisions on building relocation and any other life safety measures mandated by the situation.
Day Contacts: These individuals are authorized by your firm to call in day-to-day maintenance requests as well as special or overtime services. They should be authorized to place requests, which may involve a billback charge to your company. We ask that these contacts be limited to one or two people to ensure that we are able to respond to your requests effectively.
Lease/Rent Contacts: This individual is responsible for your office location with authorization to receive and process notifications regarding lease/rent issues.
Recipient of Invoices: This individual is responsible for receiving and approving invoices.
After Hours Contacts: These individuals are authorized by your firm to provide us with information regarding after hours activity in your space. Please list them in the order in which they are to be called in the event we need authorization for access or another situation.
Tenant’s Vendor Information
The attached Tenant Vendor Information forms request that your firm identify contractors, vendors and suppliers who regularly perform services in your premises. The information provided on the form is used to issue badges to authorized vendors and if we do not receive your completed form, your vendors and contractors may be delayed reaching your suite. It is our intent that all individuals performing work in the Building be on a list of approved vendors prior to their arrival at the Building. Additionally, the Certificate of Insurance Requirements forms should be provided to each service contractor. An accurate and complete Certificate of Insurance will be required for any company performing work in your premises.
